Coming home from the hospital can feel overwhelming.  This personalized 75-minute consultation is designed to help you transition safely and confidently from hospital to home.  During this session, we'll review your discharge instructions, medications, follow-up care and daily routines to make sure everything feels clear and manageable.  We'll focus on helping you feel supported, informed and prepared.

This appointment includes: 

• Review of discharge paperwork and care instructions

• Medication review and education

• Guidance on wound care, symptom monitoring or other recovery needs

• Help prepare for follow-up appointments

• Review if additional healthcare consultations or referrals

• Personalized tips for home safety and daily routines

• Support for caregivers and family
